Caswell Invited to Inform Regional Action Plan Focused on Rural Health Transformation
Attend an August 25 community engagement event and/or complete a needs assessment.
The Center for Thriving Communities at the UNC School of Medicine is facilitating an upcoming community listening session in Yanceyville to inform a Regional Action Plan intended to guide rural health improvement efforts and investments over the next five years as part of the Rural Health Transformation Program (RHTP).
The University of North Carolina Hospitals was selected to serve as lead agency for the North Carolina Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) Hub for Regional 4 (Alamance, Caswell, Chatham, Durham, Franklin, Granville, Johnston, Nash, Orange, Person, Vance, Wake, Warren, and Wilson counties.)
Compassion Health Care's Chief Executive Officer Billy Crumpton and Piedmont Health Services Chief Executive Officer Daniella Jaimes-Colina, PhD both serve on the Region 4 Advisory Board.
Following a regional needs assessment, the NC ROOTS Hub Leads will establish local networks of partner organizations focused on strengthening the rural health care infrastructure. Over the next five years, tailored to their regions’ needs, NC ROOTS Hubs will support rural communities by:
- Improving behavioral health and substance use services
- Enhancing care coordination across regions
- Leveraging digital tools to improve outcomes
- Expanding access to primary care
- Strengthening health care through workforce development
Community members, local organizations, healthcare providers, advocates, and other partners are invited to help identify and prioritize rural health needs, plans, and action by responding to THIS ONLINE NEEDS ASSESSMENT and/or attending a community engagement session on August 25 at Gunn Memorial Library 10am - 12pm.
During this session, participants will:
- Review key findings from our assessments of strengths and needs in NC Roots Region 4
- Discuss the health challenges and barriers facing local communities
- Identify priorities for future investment and action
- Help shape a Regional Action Plan that will guide rural health improvement efforts and investments over the next five years

Funded through a $213,008,356.47 investment from the U.S. Centers for Medicare & Medicaid Services, the RHTP is a statewide initiative led by NCDHHS designed to improve health outcomes, support sustainable access to care, and advance health care delivery for rural communities. The program will support more than 400 rural health facilities and bring hundreds of millions of dollars in federal investment to the state over the next five years.
